“…Mike Houston enters the office today for his new patient exam. His last physical exam took place while he was in college 5 years ago for football. Mike also participated in both baseball and basketball. During his earlier years, sports physicals were performed at his own doctor. His last "parent" attended physical exam was with his step-dad present in the exam room at age 14. I found out that Mike generally maintains and overall fairly healthy lifestyle. I direct him to undress completely naked and have him lay down onto the exam table.

Looking at his naked form, what I don't see is a remarkable, natural lack of upper body hair for a 26 year old. His lower body resembles a more natural look as you can discern the start of hair growth beginning at the naval and traveling down the remainder of his body. However, what is most remarkable is the size of his scrotum and each individual testis. They are actually quite large!

Lying on the exam table, Mike gave me full, un-restricted access to his body; first using a stethoscope and lastly palpating his body from his neck to his knees. His relaxed manner made it easier for me to do a prolonged supine genital examination as I was anxious to examine the contents of his scrotum before gravity came into play during a standing genital examination. I was also able to examine his perineum, noting additional hair follicles in the region.

I detected no problems with his testicles; just noting how completely they were so tightly encased by his scrotum. Standing Mike up, I continued to examine his genitalia, marveling again about the shear magnitude and now the weight of his scrotum. His pubic area is trimmed (most likely through the use of electric clippers). Turning him around, I noted how smooth his buttocks are but then noted the dense hairs buried in the crevice between the twin orbs.

Up onto the exam table and I coaxed him into the proper positioning. First some additional examination of his scrotum before prepping a thermometer for insertion. Mike's buttocks are expansive so to find his anus; I had part him open with one hand before taking aim with thermometer. !

The prostate examination was a similar experience as it took some maneuvering to get my finger inside him and to move past his sphincter. He was extremely tight as this was most definitely his first experience with anal stimulation. Turning him back around, I knew that taking a urethral culture might prove a bit uncomfortable as there were no visible signs of fluid secretions from his prostate examination. While a urethral specimen is an uncomfortable procedure, Mike permitted me to accomplish this without a disparaging word (just the customary face grimacing).

With all examination requirements, but one fulfilled, I directed Mike to continue on his own to work his 7 inch penis to ejaculation. I was able to assist with collection of the specimen…”
